Required Documents (CNA)
Government-issued photo ID + SSN
HS diploma/GED or proof of concurrent HS enrollment (min age 16)
Physical exam (within 12 months)
TB clearance (2-step PPD, QuantiFERON, or CXR if prior positive)
Immunizations: MMR, Varicella, Hep B (or waiver), Tdap, Influenza (in season), COVID-19 (if site-required)
Fingerprint-based IDPH background check
Tution Fees : $2,015
Includes: Registration: ($100, non-refundable), CPR ($65), Textbooks ($85), Supplies ($85), Fingerprinting ($60), Background Check ($55), Uniform + ID ($65).
- State exam fee: (~$85) paid directly to SIU Nurse Aide Testing.
